4 Steps to Register for Soccer!

1.  Open the Registration Form (Read all 4 steps before opening)
To open the form click on the link near the bottom of this page

2.  Type in your information
Note: You cannot save the information on the form.  Complete the form by tabbing to each text box and then typing in your information.

3.  Print
Note :  Before you print make sure your Browser's Page Setup has its margins and page orientation properly set.  This form should print out on two pages.
Page Orientation = Portrait
Left Margin = .3,  Right Margin = .3,  Top Margin = .5, Bottom Margin = .5

To print the form press the Ctrl key and the P key at the same time.  When the Print dialog box appears click OK.  A second option is to select Print from the File menu at the top of your screen.

4. Sign and bring the registration form and emergency form to one of the registration dates.
